Baby Dedication Application Form

Kindly note you will be required to attend a compulsory briefing (online/physical) held the week before the Baby Dedication service during which pastors will share the spiritual significance of this important act. Your family is also invited to a celebration luncheon with the Family Life team the week after the Baby Dedication service if you are free.  Please ensure you respond accordingly for all 3 events.
